Well, I've been banned on an almost entirely new toon. I think I've had him for a total of 30 hours perhaps. And I blame the sometimes lacking nature of Dungeonbuddy. Dungeonbuddy works great 90% of the time. But many of the paths it uses are outdated, causing you to run through mobs, or pull extra things. Examples; Gnomeregan. 100% of the time players want to jump down 2 different places in the instance, the bot doesn't. So the bot ends up running all the way, getting itself killed, over and over again. Or blackrock depths, there's a room where you can follow the wall to the right (Right before the 7 dwarf bosses in the same room) or you can go left (The longest way) the bot takes the longest, while everyone else take the fast route. This again means you wipe yourself over and over. If you're a healer, this is super annoying for a group. I guess that's how I got banned so fast. I left my healer overnight.
