After my character dies, he returns to a Medcenter and then I get these errors in the log. All I can tell you is it's a common problem, and I have no idea what causes it I know a few of us get it. One fun thing it'll do after death is tag around an NPC, so it looks really silly if decides it wants to follow a patrolling NPC around.
